Managing type 1 diabetes often means working with a team of health professionals—and Medicare offers rebates to help make that care more accessible. If you have a Chronic Disease Management Plan (CDMP) set up by your GP, you may be eligible to receive $61.80 back from Medicare for up to five allied health appointments per calendar year.

How a CDMP works:

Book a longer appointment with your GP to create a CDMP

Your GP will work with you to identify which allied health professionals will support your goals

Your plan is valid for 12 months from the date it’s created
For people living with type 1 diabetes, this team often includes:

Credentialled Diabetes Educators

Dietitians
-
Podiatrists
-
(And others, depending on your needs)

If you’re attending a session with SEE Diabetes—whether in person or via telehealth—your CDMP can be used to claim the Medicare rebate. Just bring a copy to your appointment or email it to us beforehand.

60minutes - $88.20 after rebate
30minutes - $48.20 after rebate
